Our Personalized Offerings
Post-Offer Medical Screening Questionnaire
Our pre-hire questionnaire covers various aspects of an individual’s medical history, including previous illnesses, injuries, and any ongoing health conditions. We also inquire about medication usage, allergies, and any limitations that may impact an individual’s job performance. This information helps us assess whether an applicant is medically fit for the position they have been offered.
Our goal with the post-offer medical screening questionnaire is to create a safe and healthy work environment for everyone. By identifying any medical conditions or limitations upfront, we can make reasonable accommodations or adjustments to ensure that the applicant can perform their job duties safely and without increased risk.
We understand that medical information is sensitive and confidential, and we treat all responses with the utmost privacy and discretion. The information provided is only used for evaluating an applicant’s suitability for the job and making necessary accommodations if required.
Post-Offer Medical Screening Examination
An organization’s post-offer medical screening examination plays a vital role in ensuring the health and safety of your employees. This comprehensive examination is designed to assess an individual’s medical fitness to perform their job duties effectively and without compromising their well-being.
We review all documentation provided by contracted clinics and make a determination on fitness for work. Determinations include:
- UDS
- Physical Exam
- Range of motion, balance, and dexterity
- Lift Test
Your employees will undergo a thorough evaluation of the applicant’s overall health. This includes assessing vital signs, reviewing medical history, performing physical examinations, and conducting specific tests or screenings relevant to the job requirements. We align our “passing” standards very closely with DOT standards.
The post-offer medical screening examination allows us to identify any underlying medical conditions, physical limitations, or potential risks that may affect an individual’s ability to perform their job safely. By understanding an applicant’s medical status, we can make informed decisions regarding any necessary accommodations or adjustments that may be required to support their successful integration into the workplace.
OccuMED-Line Tele-Triage
OccuMED-Line tele-triage is a distinctive service offering immediate access to expert medical advice and guidance. Through our tele-health system, individuals can connect with OccuMED1 licensed medical providers (MDs and PAs), who are available 24/7 to assess and triage medical concerns.
Whether it’s an employee experiencing a work-related injury or an individual seeking guidance regarding a non-emergency medical issue, OccuMED-Line tele-triage provides a convenient and efficient solution. Our experienced medical professionals will carefully evaluate the situation, ask relevant questions, and provide immediate recommendations based on the severity and nature of the condition. Our providers can ask more in-depth clinical questions that will better serve the injured employee in any recommendations we may make. If an injured employee is sent to a clinic or emergency room, our providers will collaborate closely with the treating provider to ensure the best care and outcomes are obtained.

Fit for Work Tele-Triage
OccuMED1’s Fitness for Work (FFW) tele-triage acts as a valuable resource. Our skilled medical professionals conduct thorough assessments, considering factors such as the individual’s medical history, current condition, and job requirements. They evaluate the individual’s functional abilities and determine if they are physically and mentally prepared to resume their job duties safely and effectively.
During the tele-triage consultation, our OccuMED1 licensed medical providers will gather relevant information and discuss any lingering concerns or limitations that may impact the individual’s ability to perform their job tasks. They provide personalized guidance and recommendations, including any necessary accommodations or modifications that may be required to facilitate a smooth transition back to work.
Our tele-triage non-work related FFW determinations include:
- Physical: employee with a new limp or employee that looks and acts sick
- Emotional: employee very anxious or irritable due to home life
- Chemical: employee with smell of alcohol or marijuana on their person
